一位80歲女性病患呈現意識改變兩天

A 80-year-old woman was presented to our gastrointestinal ward with consciousness disturbance for 2 days. She has a past history of liver cirrhosis, Child’s B, chronic hepatitis C related and hypertension. Initially, irritable mood was found which accompanied with four limbs tremor and no stool passage for 2 days. Then, consciousness exacerbated to coma despite our treatment.

一位80歲女性來到腸胃科病房呈現意識改變兩天，她有肝硬化、 C型肝炎及高血壓病史。一開始呈現躁動的情緒伴隨四肢顫動及兩天沒有排便，即便我們開始治療，意識持續惡化為昏迷狀態。

A 20-year-old woman with body weight loss, anemia, and large colon ulcer.

A 20-year-old womandenied systemic disease before. She visited our gastroenterologicoutpatient department because of recurrent periumbilical abdominal pain for three months. Besides, body weight loss from 52 kg to 42 kg was noted in this period of time. There was no fever, nausea, vomiting or obvious bowel habit change. The laboratory data revealed hemoglobin 10.3g/dL(11.3-15.3g/dL) and positive stool occult blood. The colonoscopy showed large ulcer at ascending colon around ileocecal valve. The abdominal computed tomography showed focal wall thickening around ulcer with extension out of serosa, and enlarged lymph nodes over mesocolon. Then, the patient received right hemicolectomy. The pathology report demonstrated no malignancy.

73歲男性以膽管炎表現送至急診,經膽道內視鏡引流並切片,切片診斷有中度分化不良,因而進行Ampullectomy,但術後因塑膠支架滑入總膽管,並且因手術產生接合出彎曲,而導致無法藉由傳統內視鏡方式(包含Snare, Basker, Forceps)取出,最後藉由一創新guidewire loop technique成功取出。

This 87 years old female with history of 1.Acute cholecystitis s/p laproscopic cholecystectomy more than 10 years ago, 2.Old CVA, 3.Hypertention, was admitted due to epigastragia and vomiting for 1 day. Abdominal CT at LMD showed a CBD stone, she was transferred to our hospital.

Treatment of chronic HBV：Updated in APASL

慢性B型肝炎病毒(Hepatitis B virus; HBV)感染為全球性健康的重要議題，每年均造成一百萬人口死於兩個重要的合併症肝硬化及肝癌，近年來關於HBV的繁殖，自然病程及免疫致病機轉已經被清楚的研究，因此如何剷除病毒，以防止或減少病毒的傳染，肝細胞發炎、壞死及轉變成肝硬化或肝癌是相當重要的，針對不同的藥物作用機轉，目前已有多種有效的藥物被研發出來治療慢性B型肝炎，其中傳統型干擾素、長效型干擾素及五種口服抗病毒類核藥物：干安能(lamivudine; LAM)，干適能(adefovir dipivoxil; ADV)，貝樂克(entecavir; ETV)，喜必福(telbivudine; Ldt)及惠立妥(tenofovir; TDF)已經被衛生署核准用來治療慢性B型肝炎，使用干擾素治療的優點是治療的時間固定，治療若產生HBeAg血清轉換則持久性較高，至於它的缺點則是需注射給藥及有較多副作用。反觀使用類核同質物治療的優點是口服給藥，快而有效的病毒抑制及無明顯副作用，其缺點則為治療的時間不固定，而長期服藥容易產生抗藥性的病毒，治療若產生HBeAg血清轉換時，其持久性較低，雖然慢性B型肝炎的治療，已經有多種藥物可以選擇，但是療效離理想尚遠，醫師必須考量病人的年齡、性別、肝病的嚴重度、得到療效的機率、可能發生的副作用等，考慮現有的七種核可藥品的優缺點，為個別的病人選用最適合他的藥品作為第一線治療。
